Position Summary
The Social Services Director is responsible for planning, organizing, and directing the overall operation of the Social Services Department in a long-term care facility. This position ensures that residents’ medically related emotional and social needs are identified and met in accordance with federal, state, and local regulations.

Key Responsibilities Resident Care & Support
Conduct psychosocial assessments for all residents upon admission and periodically thereafter.
Develop and implement individualized social service care plans.
Provide counseling and emotional support to residents and their families.
Assist residents in adjusting to placement, illness, and changes in health status.
Advocate for residents’ rights and ensure dignity and respect in all care interactions.
Care Planning
Participate in interdisciplinary care plan meetings.
Collaborate with nursing, therapy, dietary, and activities departments to address residents’ psychosocial needs.
Document social service interventions and resident progress in the medical record.
Discharge Planning
Coordinate discharge planning with the healthcare team.
Arrange community resources, home care services, and referrals as needed.
Educate residents and families about post-discharge support options.
Family & Community Relations
Serve as a liaison between residents, families, and facility staff.
Address resident and family concerns and assist with conflict resolution.
Maintain relationships with community agencies, hospitals, and support organizations.
Regulatory Compliance
Ensure compliance with federal, state, and local long-term care regulations.
Maintain accurate documentation required for surveys and audits.
Participate in quality assurance and performance improvement programs.
Department Management
Supervise social services staff if applicable.
Develop departmental policies and procedures.
Participate in staff education related to psychosocial care and resident rights.
Qualifications
Bachelor’s degree in Social Work, Human Services, or a related field required (Master’s preferred depending on state regulations).
Current Social Work license (if required by state).
Minimum 1–2 years experience in long-term care, healthcare, or geriatric social services preferred.
Knowledge of long-term care regulations and discharge planning processes.
Strong communication, counseling, and conflict-resolution skills.
