Director of Client Navigation
Location: Newport News, VA
Employment Type: Full-Time | In-Person

Position Summary

We are seeking an experienced Director of Client Navigation to support the coordination of care across multiple treatment facilities. While the role is primarily based in Newport News, VA , it will require approximately 25% travel supporting our facilities in Newport News, Radford (VA), and Concord (NC) to ensure clients move successfully through each level of care.

The Director of Client Navigation plays a critical role in identifying gaps in the care continuum, improving coordination between treatment programs, and ensuring clients receive the support they need throughout their recovery journey.

Responsibilities

  • Collaborate with multidisciplinary teams across facilities to identify gaps in the client care continuum and implement solutions that improve care and services
  • Track and monitor client movement between levels of care to ensure smooth transitions and continuity of treatment
  • Maintain and analyze reports and data related to client navigation and care coordination
  • Work closely with administration to identify challenges and develop strategies to improve outcomes
  • Provide updates to Admissions and Facility Management on client navigation and care progression
  • Coordinate meetings with key stakeholders to support effective transitions across programs
  • Perform additional duties as assigned to support clinical and operational goals

Leadership Responsibilities

  • Provide training and day-to-day guidance to care management team members
  • Support best practices for care coordination and client navigation across facilities

Qualifications

  • Education: Bachelor’s degree or higher required
  • Experience: Preferred: 5 years of experience in substance use disorder and/or mental health treatment settings
