JR164959## **Job Title: Social Service Designee**### **Job Summary**The Social Service Designee is responsible for assisting residents/patients and their families with social, emotional, and discharge planning needs. This role supports overall well-being by coordinating services, advocating for residents, and ensuring compliance with applicable regulations.### **Key Responsibilities*** Assess residents’ social, emotional, and psychological needs upon admission and throughout their stay* Develop and implement individualized care plans in collaboration with the interdisciplinary team* Provide counseling and emotional support to residents and families* Assist with discharge planning, including coordinating community resources and services* Maintain accurate and timely documentation in accordance with state and federal regulations* Advocate for residents’ rights and ensure dignity and respect are upheld* Facilitate communication between residents, families, and healthcare staff* Coordinate social services programs such as support groups or recreational activities* Assist with resolving conflicts or concerns involving residents and families### **Qualifications*** High school diploma or equivalent required; associate or bachelor’s degree in social work, human services, or related field preferred* Previous experience in healthcare, long-term care, or social services preferred* Knowledge of basic social service practices and community resources* Strong interpersonal, communication, and organizational skills* Ability to handle sensitive information with confidentiality* Compassionate and patient-centered approach### **Work Environment*** Typically works in nursing homes, assisted living facilities, rehabilitation centers, or hospitals* May involve interaction with elderly, disabled, or chronically ill individuals* Combination of desk work and direct resident interaction### **Preferred Skills*** Crisis intervention and problem-solving abilities* Cultural competency and empathy* Team collaboration* Time management and multitaskingPACS is revolutionizing the delivery and leadership of post-acute healthcare and support services across the country.
