Responsibilities

Develop, plan, implement and evaluate service program and service in accordance with state and federal guidelines.

Identify and provide for resident's social, emotional and psychological needs

Provide service coordination

QUALIFICATIONS

Licensed Social Worker, preferred – not required

One year of supervised social work experience in a healthcare setting, preferably with the psychosocial of the elderly

Working knowledge of nursing homes regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid

Excellent communication skills; written and verbal

Positive Can-Do Attitude
