Overview
With over 30 years of experience and established industry relationships in senior living environments, HealthDrive is the leading provider of Primary Care and Ancillary services to residents of communities nationwide. The Strategic Innovation Associate will be a key driver in transitioning our organization from volume to value. You will support the design, implementation, and scaling of innovative clinical models—including Palliative Care and Chronic Care Management—while ensuring the operational success of our Value-Based Care (VBC) strategy. This role requires a blend of project management, data literacy, and an understanding of ACO (Accountable Care Organization) mechanics. This role is a remote position. Responsibilities
Value-Based Care Operations
Attribution & Panel Management: Manage patient attribution logic to ensure accurate provider benchmarking and financial alignment within ACO and payer contracts. Gap Closure & Quality: Coordinate initiatives to identify and close "Diagnosis Gaps" and quality measure deficiencies to optimize shared savings. High-Risk Identification: Utilize data to identify high-risk patients who would benefit from specialized intervention. Partnership Support: Act as a liaison for ACO partnerships, ensuring all stakeholders are aligned on performance goals and reporting timelines. Innovative Program Execution
Program Implementation: Support the operational rollout of Advanced Primary Care Management (APCM), Chronic Care Management (CCM), and Palliative Care programs. Workflow Design: Collaborate with clinical teams to integrate innovative programs into daily workflows without increasing provider burnout. Performance Tracking: Monitor the ROI and clinical outcomes of pilot programs, providing regular updates to leadership on scalability. Qualifications
Education: Bachelor’s degree in healthcare administration, public health, business, or a related field (Master’s degree preferred but not required). Experience: 2–4 years in healthcare operations, population health, or consulting. Technical Skills: Proficiency in Excel (data manipulation), familiarity with EHR systems and/or Population Health Management tools. Knowledge Base: Basic understanding of VBC levers, healthcare operations, and process workflows. Core Competencies
