**Key Responsibilities:**

• Oversee and supervise the daily operation of the HCH Team.

• Coordinate and track new HCH enrollees, discharges, and transfers.

• Supervise Nurse Care Managers and assign NCM caseloads.

• Coordinate and facilitate health education groups and activities for clients, staff, and the community.

• Ensure timely completion and submission of the monthly Attestation report in CyberAccess.

• Serve as Cyber Access Practice Administrator, maintaining user accounts and monitoring agency usage and prescriber reports.

• Manage HIT tools and initiatives, including CyberAccess usage, CMT reports, and hospital admission/discharges.

• Facilitate staff training on Healthcare Home topics.

• Participate in monthly TA calls for HCH Directors and attend state-required meetings and trainings.

• Oversee Quality Assurance and Quality Improvement for Healthcare Home.

**Education, Experience, and/or Credential Qualifications:**

• BSN or MSN

• Registered Nurse

• Supervisory Experience

**Additional Qualifications:**

• Current driver’s license, acceptable driving record, and current auto insurance.

• Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint check.

• Completion of New Hire Orientation at the beginning of employment.

• Must complete all training requirements, including Relias, at the beginning of employment and annually thereafter.

**Physical Requirements:**

• ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.

• Repetitive movements of hands, fingers, and arms for typing and/or writing during the work shift.

• Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
