
PT /PTA- Physical Therapy

Implements plan of care as provided by the physical therapist, treating patients of all ages. Serves as a role model in delivery of professional services and as a clinical resource for staff and students.
Responsibilities
Educates patients, family, caregivers, students, and other healthcare providers using relevant and effective teaching methods.
Adjusts the treatment plan based on patient response and available evidence only after communicating with the evaluating therapist and documenting that communication in the daily note.
Produces documentation in accordance with the Clinical Documentation Policy (CTSCP67) and follows the treatment plan of care established by the PT.
Provides treatment interventions including therapeutic exercise, gait and balance training, ultrasound, electrical stimulation treatment, and aquatic therapy.
Completes daily documentation and charging in the therapy EMR.
Coordinates with the PT on patient treatment interventions.
Qualifications Licensed Physical Therapist Assistant (PTA) – Illinois Department of Financial and Professional Regulation (IDFPR).
Field of Study: Physical Therapy Assistant.
