You will identify and provide for each resident’s social, emotional and psychological needs, and support their full potential throughout their stay. You will assist in resident care planning and discharge planning, and serve as a trusted information source for residents and families. How You Will Make a Difference
Assist residents and their families with social, emotional, and family issues. Provide timely and accurate documentation to comply with facility, state, and federal policies. Participate in resident care planning. Collaborate in discharge planning and prepare residents for transition. Serve as a general source of information for residents and families. What You Need To Make a Difference
Bachelor’s or Master’s degree in Social Work or Psychology. 3+ years of experience in social services, directly with individuals, preferably in a healthcare setting.
