The Social Services Director (SSD) is responsible for planning, developing, organizing, implementing, evaluating, and directing social service programs. Must assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis; safeguard the health, safety, and welfare of all residents of the facility. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.

Duties and Responsibilities - Social Services Director (SSD):

Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs

Identifying individual social and emotional needs

Developing and maintaining individualized care plans

Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay

Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Qualifications/Requirements - Social Services Director (SSD):

BSW or MSW preferred, or Bachelors degree in related field

At least one year experience in Social Work field required

Experience in long term care and/or sub-acute setting preferred
