Duties and Responsibilities

Follow-up with payers to ensure timely resolution of all outstanding claims, via phone, emails, fax or websites

Review and updates all patient and financial information accurately as given

Verify that information is accurate as to individual or insurance company responsible for payment of bill

Monitor all billings for accuracy, updating any that contain known errors

Monitor Medicaid/healthy options coupons to assure services are billed within expected timeframes

Bill all hospital services to primary insurer or patient correctly and within expected timeframe

Follow up with insurance companies on all assigned accounts within expected timeframe

Explain hospital regulations with regard to methods for payment of accounts and maintains complete working knowledge of insurance regulations and hospital insurance contracts

Identify and report underpayments and denial trends

Analyze, identify and resolve issues causing payer payment delays; Initiate appeals when necessary

Manipulate excel spreadsheets and communicate results

Meet and maintain daily productivity and quality standards established in departmental policies

Act professionally, cooperatively and courteously with patients, insurance payors, co-workers, management and clients

Perform special projects and other duties as needed by the management team

Maintain confidentiality at all times

Other duties as assigned

Use, protect and disclose patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards

Understand and comply with Information Security and HIPAA policies and procedures at all times

Limit viewing of PHI to the absolute minimum as necessary to perform assigned duties

Qualifications

High School Diploma or equivalent required

Medical Billing and Coding certification preferred, but not required

Experience in Hospital/Facility billing required

Experience with Medicare/Medicaid follow up required

2-3 years’ experience in insurance collections, including submitting and following up on claims

Basic knowledge of healthcare claims processing including: ICD-9/10, CPT and HCPC codes, as well as UB-04

Ability to use various workflow system and client host system such as STAR, SMS, EAGLE and EPIC, as well as other tools available to them to collect payments and resolve accounts

Working knowledge of the insurance follow-up process with understanding of the fundamental concepts in healthcare reimbursement methodologies

Understanding of government, Medicare and Medicaid claims

Proficiency with Microsoft Office Suite including Excel and Word

Basic math and typing skills

Strong interpersonal skills, ability to communicate well at all levels of the organization

Strong problem solving and creative skills and the ability to exercise sound judgment and make decisions based on accurate and timely analyses

High level of integrity and dependability with a strong sense of urgency and results oriented

Excellent written and verbal communication skills required

Gracious and welcoming personality for customer service interaction

The Patient Accounts Representative

Hospital Billing and Follow-up ensures the submission of timely and clean Inpatient/Outpatient claims to the various government/non-government payers and takes the appropriate action to resolve claim issues in order to accelerate cash collections.\nEssential Duties and Responsibilities:\nSubmit Inpatient/Outpatient electronic and paper claims (UB-04 and 1500) to the appropriate government and non-government payers.\nUnderstand how to resolve Billing errors and/or warnings that are identified in the Patient Accounting and Billing System.\nKeep abreast of payer-specific and government requirements and regulations.\nEnsures claim information is complete and accurate in order to accelerate cash collections.\nAnalyze information contained within the Patient Accounting and Billing system to make decisions on how to proceed with the billing of an account.\nProcesses rejections by correcting any billing error and resubmitting claims to government and non-government payers.\nPlace unbillable claims on hold and properly communicate to various Hospital departments the information needed to accurately bill.\nProcess late charge claims in the event that charges are not entered in a timely fashion by Hospital Departments.\nSubmit corrected claims in the event that the original claim information has changed for various reasons.\nPerform the billing of complex scenarios such as interim, self-audit, combined, and split billing etc.\nLimit the number of unreleased claims by reviewing all imported claims and either billing or holding the claim for further review.\nMeet Billing productivity and quality requirements as developed by Leadership.\nMeasured on high production levels, quality of work output, in compliance with established CRH's policy and standards.\nRecord or generate revenue by gathering and processing information that impacts the patient revenue process.\nReview patient financial records and/or claims prior to submission to ensure payer-specific requirements are met.\nFollow up on unprocessed or unpaid claims until a claims resolution is achieved.\nGenerates letters to insurance or patients as needed in order to resolve unpaid claim issues.\nWorks on and maintains spreadsheets by sorting/adding pertinent data.\nAnalyze information contained within the billing systems to make decisions on how to proceed with the account.\nWork independently and has the ability to make decisions relative to individual work activities.\nIdentify comments in the billing systems by using initials and using approved abbreviations for universal understanding.\nKeep documentation clear, concise, and to the point, while including enough information for a clear understanding of the work performed and actions needed.\nCreate appropriate documentation, correspondence, emails, etc. and ensure that they are scanned to the proper account for accurate documentation.\nRead, understand, and explain benefits from all payers to coworkers, physicians, and patients.\nMake phone calls, use the internet, and send mail to payers for follow-up on unprocessed claims, incorrectly processed claims, or claims in question.\nDevelop relationships with customers/patients/co-workers in order to gather and process information or resolve issues in order to receive accurate reimbursement and optimize internal and external customer satisfaction.\nPost accurate adjustments as appropriate per billing policies and procedures, payer explanation of benefits, and the management directive.\nMaintain work procedures pertinent to the job assignment.\nAccountable for individual work activities.\nResolve questions that arise regarding correct charging and/or other concerns regarding services provided.\nComplete cross-training, as deemed necessary by management, to ensure efficient department operations.\nReport potential or identified problems with systems, payers, and processes to the manager in a timely manner.\nComplete special project assignments in a timely fashion.\nFollows HIPAA guidelines in order to maintain strict confidentiality of all patient financial and hospital information at all times.\nPerform other duties as assigned.\nEducation and Experience:\nMinimum Required Education: High school diploma or equivalent\nPreferred Education: College courses or associate's degree\nExperience: 3+ years as a Hospital Biller or Follow-up representative preferred.
