$70,000–$75,000 annually

Hawthorne Inn Lakeland, a premier Assisted Living and Memory Care community in the heart of Lakeland, FL, is seeking a

licensed and experienced LPN

to join our leadership team as the

Director of Wellness . This role plays a key part in ensuring exceptional resident care, regulatory compliance, and strong team collaboration.

The

Director of Wellness

is responsible for overseeing all aspects of resident health and wellness services, providing leadership and clinical support to the care team, and ensuring high-quality, person-centered care. This position is

on-call 24/7 with the assistance of the RCC

and works closely with the

Administrator

for additional support.

Requirements:

Current LPN/RN license in the state of Florida (required)

Assisted Living Facility (ALF) experience is required

At least one year of experience in senior living, home health, or post-acute care

Prior management or supervisory experience preferred

Strong leadership, communication, and organizational skills

Key Responsibilities:

Serve as a resident and family advocate, ensuring quality outcomes and positive experiences

Coordinate and oversee resident care plans, assessments, and wellness programs

Communicate effectively with residents, families, physicians, and staff

Monitor and document changes in resident condition and care needs

Provide training, coaching, and support to care team members

Participate in staffing coordination and respond to emergent needs when necessary

Collaborate closely with the Administrator to maintain compliance and operational excellence

Job description The Director of Wellness is accountable for all planning, organizing, directing, and evaluating of the care department. Ideal candidates must have a strong managerial background in developing team members and track record of driving results in making a quality wellness program for our residents. The Director of Wellness will oversee budget and census management that assists with the community’s overall goals. The Wellness Director respectfully interacts with all residents, family members, visitors, colleagues, and staff.

Qualifications

Must be a graduate of an accredited school of nursing and must have and maintain a valid, unrestricted nursing license in the state, preferred.

Minimum of three (1) years successful leadership role in a community or long-term care setting.

Experience with aging adults and knowledge of regulations pertaining to Assisted Living Facilities/Memory Care preferred.

Ability to react calmly and confidently under pressure and in unexpected situations.

Essential Functions

Oversee the recruitment, hiring, training, supervision, and evaluation of all care associates to assure quality services.

Oversee the planning and delegation of work assignments, ensuring effective scheduling including covering call offs or working the floor as needed.

Provide case management services to residents, ensuring completion of the pre- occupancy screening, assessment/reassessment, and the service plan.

Investigate resident and associate incidents, identify safety hazards, and institute corrective actions in a timely manner.

Monitor and maintain records reflecting resident’s physical and mental condition. Report significant changes in condition to the Executive Director/Administrator and physician.

Ensure compliance with regulatory agencies.

Operate department within budgetary guidelines.

Serve as Manager on Duty (“MOD”) as assigned.

Attend and participate in in-service educational classes and on-the-job training

Perform all other duties as assigned.

Physical and Sensory Requirements Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. Must be able to push, pull, move, and/or lift a minimum of 50 pounds to chest level and be able to push, pull, move, and/or carry such weight a minimum distance of 5 feet. May be necessary to assist in the evacuation of residents during emergency situations.
