Overview

The Prince William Health District (PWHD) operates multiple public health programs in Prince William County, Manassas City, and Manassas Park. PWHD is a progressive public health leader providing services that promote population health, protect public health and the environment, and ensure residents\' access to health services and information. We are seeking a dynamic and innovative individual to plan, implement, and evaluate community-based population health improvement programs through education, outreach, and engagement. This position will conduct these activities through internal collaboration and external partnerships within the Population Health and Disease Prevention Division of PWHD. About This Role: The Community Health Educator supports PWHD\'s efforts to develop and implement strategies to improve population health. This position represents the health district at community events and coordinates with other departments and partners to identify and implement outreach and health education through community activities. This role also supports and reinforces healthful behaviors by working with internal team members, community coalitions, schools, businesses, city leadership, urban designers, and other local organizations and partners to build support for lifestyle change. Lead, support, and develop strategies focused on health communication efforts targeting diverse audiences and coordinating engagement opportunities with community partners and health coalitions to improve the health of the PWHD community. This position functions as an expert in health education and supports PWHD\'s accountability to social determinants of health. The role prioritizes population health focused strategies designed to support optimum wellness and a healthy environment among people who live, work, and play in Prince William County, Manassas City, and Manassas Park. Minimum Requirements: High school diploma or G.E.D. and 6 years of related experience with public health and health education. Preferences: Certified Health Education Specialist Certification We seek candidates with the ability to problem-solve, set priorities and work independently, who possess excellent interpersonal, oral, and written communication skills — essential to effectively interact with internal and external customers to promote public health. Bachelor\'s degree in public health or related subject such as health education and/or equivalent public health education experience Convene public health partners, communities, and individuals—especially those most impacted by disparities and inequities—to conduct health assessments to identify health priorities and root causes of health disparities Plan, implement and evaluate community-based population health improvement programs with partners and team members Lead development of internal/external disease prevention and wellness promotion curriculums Identify, implement and evaluate opportunities to improve health; partner in the formation of healthy activity programs/opportunities Coordinate and schedule program activities with groups interested in making communities healthier Support alliances to broaden the response to health concerns; including attending/conducting meetings/training sessions Advance the work of community coalitions and various committees to improve community health and promote health throughout PWHD May co-chair community healthcare coalitions Establish effective relationships, engage new partners and facilitate collaboration among multidisciplinary stakeholders and volunteers throughout the PWHD community Develop seminars, learning sessions or other community trainings related to health data May provide support in writing, editing and submission of reports on goals and outcomes Verifies data for accuracy and identifies trends, develops projections and prepares reports May support the preparation of project and training work plans for PWHD and related agencies Supports supervisor as a liaison for PWHD and partner agencies on public health matters Supports effective community health assessment/improvement planning processes Builds effective working relationships; takes initiative on projects and seeks opportunities to improve health May be required to perform other duties; may include functions in support of an emergency declaration Able to work evenings and weekends on occasion Special Requirements: Experience with public health, health education, and community outreach. Prince William is diverse and needs experience with diverse stakeholders. A valid driver\'s license with safe driving record is required. Passage of local, state and federal background checks prior to employment are required. Work Schedule: The position includes field work with a full-time schedule and variable days/hours to support community engagement activities (examples include community outreach events, outbreak investigations, clinical/immunization events, or public health emergency response activities). Hiring Salary Range:
