Position Overview:

The Bilingual Community Health Worker (CHW) supports Heritage Health Network’s Enhanced Care Management (ECM) program by identifying and engaging Medi-Cal members who may qualify for care management services. This role conducts telehealth outreach to members, screens individuals for ECM eligibility, verifies eligibility requirements, and submits referrals to health plans. CHWs also conduct community outreach at locations such as food banks, shelters, churches, probation offices, clinics, and hospital discharge departments to identify individuals who may benefit from ECM services. Spanish language proficiency is required to engage effectively and support the communities served.

Conduct telehealth outreach to members 3–4 days per week to educate them about ECM services

Screen individuals for ECM eligibility

Verify member eligibility requirements

Submit ECM referrals to health plans

Document outreach activities and member interactions in organizational systems

Conduct community outreach activities 1–2 days per week

Attend community locations including food banks, shelters, churches, probation or parole offices, clinics, and hospital discharge planning departments

Build relationships with community partners to support referral generation

Educate community members about ECM services and eligibility

Assist individuals in connecting with healthcare and community resources

Participate in team meetings and outreach coordination activities

Requirements

Education:

High school diploma or equivalent required

Specialty area:

Community health, outreach programs, social services, or care coordination

License:

Valid Driver’s License

Community Health Worker (CHW) certification preferred but not required

Experience:

Previous experience in community outreach, social services, healthcare navigation, or member engagement preferred

Willingness to travel for home visits and community engagements.

Bilingual Spanish and English communication skills

Strong community outreach and engagement skills

Ability to communicate effectively with diverse populations

Strong organizational and time management skills

Basic computer proficiency including email and documentation systems
