Job Summary The Outreach and Enrollment Representative works to provide outreach opportunities for community residents who lack access to healthcare and health insurance, providing enrollment and application follow‑up. It increases health insurance coverage and access to quality primary health, dental, and behavioral care for community residents.
Reports to: Outreach and Enrollment Manager Supervision Exercised: None
Education / Experience / License / Certification
Must have a High School Degree or GED
Two or more years of office and/or customer service experience is required. Experience in a healthcare environment and/or working with programs and services intended for low‑income, underserved populations is preferred.
An equivalent combination of post‑secondary education and experience may be considered.
Valid driver’s license and auto insurance as required by law
Required Travel: Occasional travel may be required including day‑to‑day local travel.
Knowledge / Skills / Abilities
Strong ability in leadership, advocacy, communication skills both written and verbal, resource research, creative problem solving, and sensitivity/responsiveness to varying cultural characteristics and beliefs.
Conversational Spanish preferred.
Ability to work independently.
Excellent interpersonal skills.
Ability to understand verbal information, to exchange verbal information with others, and to present information verbally.
Ability to explain and summarize detailed concepts like Medicaid enrollment process or essential health benefits in a way the general population can understand.
Capacity to read and understand written materials and to compose information/instruction in written form.
Capacity to prioritize and be organized.
Knowledge of organizational policies, procedures, systems, and objectives.
Mathematical reasoning and analytical and problem‑solving skills.
Computer skills including database utilization, electronic health records systems, and MS Office applications.
Essential Duties and Responsibilities
Embrace the philosophy, mission, values, and Communicate with Heart service model supported by the Board of Directors of Pueblo Community Health Center, Inc.
Participate in training and educational opportunities related to the Affordable Care Act, Marketplace, CHP+, and Medicaid enrollment standards and competencies.
Maintain expertise in eligibility and enrollment rules and procedures, health plan options, insurance affordability programs, underserved and vulnerable population needs, and privacy and security standards.
Conduct "in reach" outreach with currently uninsured health center patients and "outreach" to non‑health center patients.
Adhere to the guidelines and procedures of PCHC.
Schedule patient registration appointments and register patients to PCHC on the correct insurance programs or discount programs.
Determine eligibility for CICP, Family and Adult Medicaid, and CHP+ Programs.
Uphold CICP, CHP+, Family and Adult Medicaid, and Medicaid and CHP+ presumptive eligibility rules and regulations.
Assist patients in accessing the medical system and inform them of PCHC guidelines for using medical and dental services.
Maintain patient records to meet audit requirements for CICP, primary audit and PE.
Maintain departmental statistics as directed and participate in departmental and general staff meetings.
Perform other related duties as assigned.
Physical Demands
Frequent communication with peers and the public while performing routine office tasks and exchanging accurate information.
Frequent retrieval and dissemination of information using written materials and electronic devices.
Frequent observation, receipt, and analysis of information from all relevant sources, identifying information by categorizing, recognizing differences or similarities, and detecting changes in circumstances or events.
Frequent operation of computers and other office productivity equipment.
Frequent stationary position while performing routine office tasks.
Frequent movement around the office, positioning self while performing routine duties such as accessing files and operating equipment.
Frequent handling of objects up to 25 pounds while performing routine duties such as opening file drawers and moving patient files.
