
Community Health Representative (CHR)

Definition
The Community Health Representative (CHR) performs para‑professional community health work that supports clinical operations and relates to direct health care. The CHR works as an integral part of a community‑networking system, carrying out activities within the scope of the Tribal CHR Program to provide services to patients within the community and assisting in clinics and other health care programs serving the community.
Duties and Responsibilities
Serves as an extension of the tribal health clinic to provide services within CHR scope to home‑bound, elderly and/or debilitated patients.
Provides homemaker services to patients in need of assistance, which may include cooking, shopping, cleaning and bathing home‑bound or orderly patients.
Conducts routine home visits to observe/assess patients and conveys patient needs back to the tribal health clinic.
Assists with identifying/coordinating patient services with Tribal Social Services and/or Housing.
May be requested to deliver medications to home‑bound patients using the five rights of medicine administration.
Prepares activity reports to the Health Clinic Administration/Community Health Nurse.
Coordinates patient referrals for the tribal health clinic and Community Health Nurse.
Takes and records vital signs, collects specimens, urine and throat cultures as directed by clinical staff.
Provides transportation to elderly or debilitated person(s) who cannot transport themselves or who lack other means of transportation.
Maintains current immunization status records and participates in the EMS First Responder System.
Applies First Aid to patients served (as needed) and arranges for higher‑acuity care if required.
Assists with conducting community programs and conveys pertinent clinic information to clients for health‑related topics.
Ensures patient safety when assisting elderly, debilitated and/or limited‑mobility patients in and out of CHR vehicles.
Ensures patient safety for patients who may be compromised due to side effects of medications.
Knowledge, Skills and Abilities
Knowledge of resource agencies available to the community and the native community.
Ability to provide home health care under supervision; deal with program clientele and resource personnel; communicate program services and goals; maintain accurate client records while protecting confidentiality and work effectively within the Native community overall.
Ability to critically evaluate and act appropriately upon acquiring information.
Ability to react to life‑threatening situations and communicate effectively with a variety of people under stressful conditions.
Interpersonal, planning and evaluating, problem‑solving and technical competence skills.
Must have a valid Nevada Driver's License, clean driving record, and be insurable under the Tribe's insurance policy.
Must provide a copy of a Nevada DMV report.
Must obtain training and comply with HIPAA requirements at all times.
May be required to work evenings, nights, weekends, holidays, or rotational/call‑back basis in instances of emergent need.
Must successfully pass a background check.
Must be able to pass drug and alcohol testing in accordance with Substance Abuse Policy.
Qualifications and Experience Requirements
High school diploma or GED.
Two years working in a health‑related field, community health or in‑home health setting.
