CommuniCare is a rapidly growing company, with 170 centers across IN, MD, MO, OH, PA, VA, WV**Benefits**:* Competitive Wages, No Wage Freeze* Generous PTO, End of Year PTO Buy Back* Work Schedule Flexibility, Work-Life Balance* Promote-from-within culture; In-house stability and continuity of care* Paid CEU Online Library**Qualifications:*** Must be a state licensed clinician (PT, OT, SLP, PTA, or COTA) from an Accredited Program.* Excellent oral and written communication, and effective leadership skills. **Responsibilities include but are not limited to:*** Performs Evaluations, Screens, and treatments of facility patients/residents who are in need for Therapy services.* If an evaluating therapist, perform patient/resident evaluation under physician order and develops a treatment plan in accordance with patient’s needs.* Implements treatment plan through direct treatment, education of treatment staff which includes therapists and other members of team involved in the treatment plan.* Attends/leads interdisciplinary meetings as appropriate, as well as function as a resource for clinical programming and caseload.

Will also relay clinical findings and make recommendations to other members of the rehab team.* Collaborates with facility and corporate Denials Management Specialist regarding any ADR/denied therapy claims.* Conducts staff/family training and education when appropriate by formal and informal in-services as well as consultations.* Maintains accurate billing information in accordance with facility procedures daily, including training/re-training of team members, and complies with documentation requirements for rehab department. #J-18808-Ljbffr
