Key Responsibilities Provide independent care for patients with acute and chronic illnesses, focusing on older adults.
Take full accountability for patient care, collaborating with specialists as necessary while remaining engaged in their treatment.
Work closely with hospitalists for hospitalized patients, regardless of employing organization.
Manage assessment, diagnosis, treatment, education, health promotion, and care coordination for complex health needs.
Lead a care team, including a medical assistant, care facilitator, and care coordinator.
Engage with transitional care teams for patients unable to visit the office, coordinating with case managers and other healthcare providers.
Facilitate weekly Super Huddle and Transforming Care meetings, providing leadership in the center alongside management.
Support recruitment efforts for both patients and healthcare providers within the center.
Mentor and coach PCPs to achieve outstanding clinical outcomes, actively participating in team huddles.
Collaborate with operational leaders to enhance center financial performance and membership growth.
Perform additional duties as assigned based on management directives.
Qualifications MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related specialty required.
Current and active medical licensure is mandatory.
A minimum of one year of clinical experience in geriatrics or family practice preferred.
Completion of ChenMed training, including Masterful Conversations, is part of the professional development plan.
Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty is preferred, required for eligibility.
Must possess a valid DEA number for schedule II-V controlled substances.
Basic Life Support (BLS) certification required within the first 90 days of employment.
