Description Winchester Gardens is seeking a Director of Social Services to join our team!
Winchester Gardens is a Springpoint Senior Living community located in Maplewood, NJ. We are seeking a Full-Time Director of Social Services to join our team. At Springpoint, our Director of Social Services primary purpose is to provide medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial wellbeing of each resident. “Medically-related social services” means services provided to assist residents in maintaining their ability to manage their everyday physical, mental and psychosocial needs.
Responsibilities
Develop and implement case management services to all residents
Make arrangements for obtaining needed adaptive equipment, clothing and personal items
Maintain contact with family (with resident’s permission) to report on changes in health, current goals, discharge planning and encouragement to participate in care planning
Maintain progress notes under federal and state guidelines for Long Term Care
Assist staff to inform residents and those they designate about the residents’ health status and health care choices and their ramifications.
Make referrals and obtain services from outside entities (e.g. talking books, absentee ballots, community wheelchair transportation)
Assist residents with financial and legal matters (e.g. applying for pension, Medicaid, referrals to lawyer, obtaining Power of Attorney, legal guardianship procedures, trustee)
Discharge planning services (e.g. facilitates transfers to other facilities, helping to place resident on a waiting list for community services, arranging intake for home care services for residents returning home, arranging for home care).
Arranging for provision of needed counseling services
Through the assessment and care planning process, identify and seek ways to support resident’s individual needs and preferences, customary routines, concerns and choices
Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents’ individual needs.
Facilitate family and resident support groups
Education and Experience
CSW or LSW required.
Minimum three (3) years experience with seniors
Experience in Long Term Care, state surveys and maintaining occupancy
