We are seeking a compassionate, experienced, and dedicated
Social Services Director
to join our skilled nursing facility leadership team. This confidential opportunity is ideal for a professional who is passionate about resident advocacy, strong clinical collaboration, and ensuring exceptional psychosocial care within a long-term care setting. Key Responsibilities: Lead the Social Services Department and oversee resident psychosocial care planning Ensure compliance with all federal, state, and facility regulations (including CMS and survey readiness) Conduct and coordinate resident assessments, care plans, and discharge planning Provide emotional and social support to residents and families Collaborate with interdisciplinary teams including nursing, administration, and therapy Manage admissions, referrals, and resident transitions Support grievance resolution and resident rights advocacy Supervise and support Social Services staff, if applicable
Qualifications:
Bachelors or Masters degree in Social Work (MSW preferred) Previous experience in a skilled nursing facility or long-term care setting required Strong knowledge of Medicare/Medicaid regulations and survey process Excellent communication, leadership, and organizational skills Ability to work effectively in a fast-paced healthcare environment
