Director Of Wellness
HOLI Senior Living has a rare opening for a Director of Wellness RN or LPN. The Director provides clinical oversight and leadership to the senior living community wellness team, while partnering with the Executive Director. This role allows an RN or LPN to develop and implement systems and processes to deliver outstanding services to our senior living residents. Must satisfy all state health, licensure/certification, and background requirements. Education/Experience: Must have an RN or LPN license in the State of Oregon. Prefer assisted living experience. Demonstrates leadership skills in applying nursing and rehabilitative care techniques and has in-depth knowledge of applicable state and Federal rules, regulations, and standards. Currently licensed or eligible for license as RN, plus any additional licensing requirements in state(s) of practice. Essential Duties and Responsibilities: Coordinates wellness program with resident, family, physicians, consultants and community team for total resident care. Assesses potential residents prior to move-in and residents returning from hospital or skilled nursing facility. Routinely visits residents to perform ongoing assessments and determine need for additional or modified services. Develops plans of action for nurses, medication technicians, and caregivers based upon resident need. Supervises recordkeeping of wellness and medical treatments and related services. Ensures availability of supplies and equipment related to wellness care. Monitors resident incident reports and medication error reports to assess causes and preventative measures. Trains all community team members in techniques developed to maintain resident independence and higher functioning. Documents training and conducts refresher training session to maintain appropriate skill sets. Evaluates team member skills by observation and implementing corrective action where necessary. Organizes and ensures completion of Community Quality Improvement Program, including but not limited to fall tracking, hospital readmissions, medication errors, etc. Acts as liaison between community Wellness team, other community teams, consultants, and physicians. Monitors Wellness budget, including labor. Ensures appropriate staffing for Wellness Department, schedules wellness staff on all shifts according to budget guidelines, and recommends changes to Executive Director. Recommends changes to Wellness policies and procedures to Executive Director and Vice President of Clinical Services. Participates in continuing education by attending in-services, seminars, and reading current literature. Studies and analyzes techniques in resident care, which will motivate and move residents towards greater independence, as well as higher levels of functioning. Acts as profession leader, role model and innovator in the community. Welcomes and interacts with newly arriving residents to achieve a successful transition. Keeps Executive Director informed of resident care activity, team member concerns, staffing patterns and other relevant topics. Assists the team with passing medications and provision of resident care as needed; May include picking up shifts due to limited availability of staff. Practice basic infection control with linens, hand washing, and in all other aspects of delivery of resident care. Attends mandatory meetings as scheduled by Executive Director. Maintains confidentiality of resident, community, employee, and company information.
