Develop, plan, implement, and evaluate social service programs in accordance with state and federal regulations Identify and address residents' social, emotional, and psychological needs Coordinate services to support resident well-being and successful care outcomes Collaborate with interdisciplinary team members to ensure holistic resident care Maintain compliance with nursing home regulations, including MDS procedures and Medicare/Medicaid guidelines What You Bring
Licensed Social Worker preferred (not required) Minimum of one year of supervised social work experience in a healthcare setting Experience working with the psychosocial needs of the elderly, preferred Knowledge of nursing home regulations and reimbursement guidelines Strong written and verbal communication skills Positive, dependable, and team-focused attitude Excellent attendance and professionalism Benefits
