About the Role

The Company is seeking an Executive Director for Home Health and Hospice services. The successful candidate will be tasked with leading and enhancing these services, ensuring the highest standards of care and compliance with all regulatory requirements. This pivotal role involves the overall management, development, operations, and evaluation of home health and hospice services, with a strong focus on mentoring and developing staff, coaching on best practices, and strengthening team performance. The Executive Director will also play a key role in supporting the transitions of care initiative, particularly in the area of reducing hospital readmissions, and will be a vital partner to the IPA team.

Applicants for the Executive Director position must have a BSN and a Master’s degree in a healthcare-related field, with an active RN license. National certification as a Home Health Manager, OASIS, or ANCC certification in a relevant specialty is required within one year of hire. The ideal candidate will have a minimum of 5 years' of home health experience, with hospice experience preferred, and at least 3 years' of leadership experience. The role demands a transformational, servant leader with a hands-on approach, high emotional intelligence, and strong executive presence. Deep clinical and operational expertise in home health and hospice, particularly within an agency setting, is essential for this position.
