Independent Living Advocates

to support individuals through two core programs serving youth and adults in the Quad Cities area. Youth Transition Advocate

works with disabled youth between the ages of 14-21 with an IEP, 504 plan, or a documented disability in Jr. High School and High School. The advocate introduces and encourages them to explore future career paths such as attending college or going into the skilled trades. They also learn life skills such as budgeting, workplace readiness, job interview skills and resume writing. The advocate would be responsible for contacting area schools to arrange a time to discuss the program and gauge interest. They would need to develop lesson plans, curriculum, plan field trips to area colleges and businesses, line up speakers for various topics related to the work world, and possibly set up some job shadowing or attending job fairs with the students. They would also be responsible for distributing and collecting the required paperwork and signatures from the student and their family, entering the information into the database, filling out required paperwork, and submitting monthly billing. Nursing Home Transition Advocate

works with people in Illinois nursing homes that are between the ages of 18-59 who are able to move back into the community. The advocate would visit area nursing homes to find applicable candidates for this program. Once a person (consumer) is deemed eligible, the advocate would be responsible to develop a relocation plan and identify needed community supports to ensure successful transition. The advocate would also be responsible for providing continued supports for 90 days after the move. The advocate would be responsible for filling out all required paperwork and entering information into the appropriate database. Both roles are Full Time (40 hours per week) and have a salary between $42,000-$45,000 (depending on experience) with full benefits (health, vision, and dental) for the employee. After one year, the employee is able to contribute to the company 401K. The company contributes a 3% match of employee contributions.
