Director of Wellness and Case Management Reports To: Executive Director

Position Type: Part-Time 20-25 hours a week (Exempt)

Location: Delaware County/ Hybrid options available

Position Summary The Director of Wellness and Case Management Programs for Centro de Apoyo Comunitario (CAC) provides leadership, strategic direction, and oversight for the organization’s emotional health, wellness, and case management initiatives. This role ensures that programs are culturally and linguistically responsive and accessible to immigrant and multilingual communities.

The Director supervises program staff, manages partnerships with mental health providers and community organizations, and leads the development of innovative services that address barriers to care such as language access, stigma, and immigration-related stress. The position plays a key role in program development, grant implementation, and evaluation.

Key Responsibilities Program Leadership and Strategy

Lead the design, implementation, and evaluation of mental health and case management programs.

Develop culturally responsive and trauma-informed services tailored to immigrant and refugee communities.

Identify emerging community needs and design new program initiatives to address them.

Ensure programs align with organizational mission, strategic priorities, and funder requirements.

Staff Supervision and Team Development

Supervise case managers, community health workers/promotores, and program coordinators.

Provide training and support in trauma-informed care, mental health first aid, and culturally responsive service delivery.

Foster a collaborative and supportive team environment.

Case Management and Service Coordination

Oversee case management services supporting individuals and families with social, health, and mental health needs.

Develop systems for intake, assessment, referral, and follow-up.

Build referral networks with mental health providers, clinics, and social service agencies.

Ensure high-quality documentation and ethical standards of care.

Provide direct follow up to families experiencing crisis.

Oversee case management partnership with Murphy’s Giving Market (MGM)

Supervision of student internships for CAC and MGM

Community Partnerships and Outreach

Cultivate partnerships with mental health providers, hospitals, clinics, academic institutions, and community organizations.

Represent the organization in community coalitions and mental health initiatives.

Actively participate in efforts to safeguard the wellness of the local immigrant community.

Promote awareness of mental health resources and reduce stigma within immigrant communities.

Grant and Program Management

Manage grant-funded programs and ensure compliance with reporting requirements.

Track program outcomes and evaluate impact using data and community feedback.

Contribute to grant proposals and reports related to mental health and wellness programming.

Organizational Leadership

Collaborate with the Executive Director and leadership team on strategic planning.

Support organizational advocacy related to immigrant health and access.

Contribute to organizational growth and sustainability efforts.

Qualifications Required

Master’s degree in social work, public health, counseling, psychology, or a related field.

Minimum of 5 years of experience in mental health, social services, or community health programs.

At least 2–3 years of supervisory or program management experience.

Experience working with immigrant, refugee, or multilingual communities.

Knowledge of trauma-informed care and culturally responsive mental health practices.

Strong organizational, leadership, and program management skills.

Preferred

Clinical license (LCSW, LPC, LMFT, or equivalent).

Bilingual or multilingual skills (Spanish strongly preferred).

Experience managing grant-funded programs.

Familiarity with community health worker or promotor/a models.

Cultural humility and commitment to immigrant community empowerment

Strong partnership and coalition-building skills
