Job Description

Schedule: Full Time, Sunday - Thursday (9:00 am - 5:00 pm – variable hours) Job Summary: The Director of Health Services (DHS) embodies Yoi Shigoto by leading the overall implementation of health and wellness services in the community. The DHS serves as a liaison with families, hospital personnel, physicians, community organizations, and other related service agencies to provide quality care to MBK Senior Living residents. The DHS is crucial for establishing MBK Senior Living as the senior living provider of choice. Additionally, the DHS is responsible for hiring, training, managing, and leading a care team that provides superior care while maintaining the physical, psychological, and emotional wellness of residents, in accordance with MBK’s core values. Independence, dignity, autonomy, and purpose needs of residents are integrated into each resident’s plan of care. Residents and their loved ones are included as partners in the development of individualized service plans. Essential Job Duties

Conduct and coordinate potential resident assessments in accordance with current rules, regulations, and community policies. This includes ensuring an initial assisted living resident assessment prior to each resident’s move‑in date and a comprehensive care plan upon move‑in. Invite memory care director (if applicable) to participate in assessments for memory care residents to ensure completion of lifestyle survey and gathering of psychosocial information. Consult with regional director of health and wellness and AFG nurse case manager for potential residents with suspected prohibited or complex conditions. Draft initial service plans for new residents; review with responsible parties and Executive Director. Establish service plans that promote independence and dignity; evaluate residents through a whole‑person wellness lens. Conduct routinely scheduled re‑assessments, update service plans, and obtain required signatures per policy. Delegate service plan process to memory care director as appropriate. Alert Executive Director, family members and others of any changes in resident health and community capacity to meet those needs under licensure. Ensure continuity of residents’ total wellness by coordinating health and wellness services with other resident services. Work closely with all departments to ensure residents receive all available services to enhance quality of life and promote total wellness. Act as a resource and care manager for residents and their families. Contact physicians directly or through family to obtain orders for care. Maintain effective communication with family and service providers. Meet regularly with the director of wellness programming and memory care director/wellness program coordinator to ensure a robust array of program offerings. Meet regularly with the executive chef to ensure resident special diets are followed and up‑to‑date. Participate in community Continuous Quality Improvement (CQI) implementation and evaluation. Coordinate and facilitate weekly interdisciplinary roundtable discussions, including department heads and outside providers as needed. Oversee health and wellness databases and systems including wanderguard, electronic MAR, eHR system, electronic check‑in system, and pager system. Collaborate with director of environmental services to ensure best safety practices, including disaster preparedness, infection control protocols and resident elopement drills. Recruit, interview, hire, manage, schedule, motivate, coach, train, evaluate and supervise department staff in accordance with community policies and established budgets. Review timekeeping, meal breaks and overtime daily. Conduct orientation for new Team Members – cultural orientation, dementia basics, caregiver skills orientation, and medication technician training. Ensure ongoing training and education for all Team Members in accordance with governing requirements and community policies. Ensure all care staff complete state‑required training: initial caregiver orientation and ongoing monthly in‑services. Gather demonstration of caregiver and medication technician skills by observing team members and coach as needed. Supervise care and medication documentation and oversee appropriate use of resident progress notes, alert charting, end‑of‑shift reports, incident reporting, weight monitoring, behavior monitoring, medication administration records, narcotic counts, medication destruction, medication refill documentation, and all other documented health and wellness processes. Ensure care team participation in daily SPARKS meetings and lead by example. Perform administrative duties accurately and timely, including expense reporting, invoice submission, team member evaluations, necessary forms, reports, studies, and resident records. Coordinate departmental schedule to ensure adequate staffing in accordance with legal requirements and budgets. Maintain inventory of supplies necessary for resident care. Coordinate prescription orders with doctors’ offices and pharmacy delivery of medications, overseeing central storage, tracking and delivery of medications and ensuring all medication carts and records are in order. Ensure all medication administrations comply with guidelines and state regulations. Ensure overall safety, health and wellness of all residents and that all governing requirements are closely monitored and executed. Ensure physician orders are carried out properly. Offer health and wellness clinics and classes for residents—one scheduled monthly, which may be delegated to outside providers. Assist in community marketing effort through positive interactions with families and outside health service providers. Act as Manager on Duty when necessary. Attend all regional and required department head meetings and trainings including monthly wellness meet‑ups. Non-Essential Job Duties

Perform other job duties or special projects as assigned by the Executive Director. Collaborate with AFG nurse case manager and regional director of health and wellness to ensure compliance with MBK policy and procedure and State regulatory guidelines. Maintain a safe and secure environment for staff, residents, and guests following established safety standards. Understand and comply with all federal, state, and local regulations, and company policies concerning the department. Display tact and friendliness when dealing with residents, Team Members and guests. Promote teamwork and open communication in accordance with MBK principles and core values. Assist Executive Director in completing an annual budget, ensuring financial goals are met monthly and labor and other expenses align with these goals. Minimize waste and misuse of supplies/equipment. Supervisory / Management Responsibilities

Health Services Assistant – 1 to 6 employees. Caregiver – 8 to 48 employees (shared with Memory Care Director and/or Director of Assisted Living). Medication Technician – 2 to 16 employees (shared with Memory Care Director and/or Director of Assisted Living). Minimum Job Requirements

Certified RN, LPN, or LVN or 2 years of full‑time college attendance. Completed all required specialized Nursing coursework for RN, LPN or LVN certification. Maintain license including CEU completion and timely renewal. Prior management or supervisory experience in a similar assisted living role. Current First Aid certification. Background clearance and health screening with negative TB test within 6 months. Valid state driver’s license and reliable transportation for assessments. Proficiency with Microsoft Office Suite and office equipment. Excellent communication skills in English. Basic math skills for personnel actions and budgeting. Ability to make independent decisions and remain calm during stress. Ability to deal tactfully and professionally with all stakeholders. Maintain neat and organized work environment. Preferred Job Requirements

Bachelor’s Degree in Nursing, Human Services or Health Care field is highly desirable. Physical Demands

Move intermittently throughout the work day. Lift/carry and pull up to 40+ pounds. Push residents up to 250 pounds. Assist in moving residents when necessary.
