in a dynamic healthcare environment. Key Responsibilities:
Lead, supervise, and mentor the
case management team , including case managers, social workers, and utilization review staff Oversee
utilization review
processes to ensure appropriate, cost-effective patient care Direct
discharge planning programs
to streamline patient transitions and reduce hospital readmissions Collaborate closely with
physicians, nurses, and interdisciplinary teams
to ensure coordinated, patient-centered care Maintain compliance with
state and federal healthcare regulations , accreditation standards, and quality metrics Analyze patient care trends, outcomes, and operational data to drive
continuous improvement
initiatives Develop policies and procedures to enhance
efficiency, patient satisfaction, and clinical quality Qualifications / Requirements:
Graduate of an
accredited Registered Nursing (RN) program Active RN license
in Iowa or eligible through
compact/multistate licensure Minimum
2+ years of experience
in case management, utilization review, discharge planning, or related healthcare leadership roles Strong
leadership, communication, and analytical skills Knowledge of
healthcare regulations, quality metrics, and patient care best practices Experience in
hospital, acute care, or rehab settings
