well-connected and motivated Community Outreach & Intake Liaison
who understands the New Jersey DDD system and has experience working with
Support Coordinators, families, and community partners . This role is ideal for someone who enjoys relationship-building and wants to play a key role in expanding access to quality services for individuals with developmental disabilities. Key Responsibilities
Build and maintain strong professional relationships with DDD Support Coordinators throughout New Jersey Actively promote Ameriwell Care’s services and increase referral activity Coordinate and manage the intake process for individuals entering services Serve as a primary point of contact for families, support coordinators, and referral sources Track outreach efforts, referral pipelines, and census growth Represent Ameriwell Care at professional meetings, networking events, and community engagements Support smooth onboarding and service launch for new individuals Qualifications
Experience in NJ DDD, healthcare, or home care services, preferably with knowledge of the New Jersey Division of Developmental Disabilities system Strong communication and relationship-building skills Organized with strong attention to detail Comfortable working both in the office and in the field Reliable transportation for travel within New Jersey Benefits
