Position Summary

Lead patient care in eye and vision health and general well‑being.

Days of practice
Hours of operation
Patient schedule
Fee schedule
Keep 100% of exam and professional fees
For fair market value, you can opt into:
Equipment/instrumentation
Administrative and/or Opt-Tech support services (if state allows)
Minimum Qualifications
Doctor of Optometry (OD) from an accredited optometry school
Valid state optometry license
Ophthalmologist (MD or DO) from an accredited school
Valid state ophthalmology licenseGeneral Liability Certificate
Professional Liability Insurance Certificate
W‑9
ME & VA license agreements are with Gumberg Asset Management Company
