New Jersey Mentor a part of the Sevita family that provides Residential and Treatment Foster Care services to youth who may be facing out-of-home placements, transitioning back into the family home or independent living alternatives, or involved in the juvenile justice system. Through personalized programming, we help youth gain stability, comfort and confidence to overcome all challenges and live well.

Community Outreach Worker – Part Time $20/hr (20‑25 hrs/week)

Location: Camden, Burlington and Gloucester Counties

Responsibilities

Provide training, assistance, and/or supervision to individuals in daily living, personal care, financial management, community and social integration, medication administration, and job skills development.

Establish a therapeutic relationship to ensure effective guidance and support.

Work on behavioral assistance and family systems; collaborate with family members of individuals in program.

Ensure objectives are met in accordance with the service plan.

Provide transportation for individuals to activities and appointments.

Maintain progress and service notes; complete and submit documentation to direct supervisor per applicable policies and deadlines.

Provide verbal feedback and relevant data to the direct supervisor regarding individuals’ services and plans.

Meet with employers to develop potential work opportunities for individuals receiving services.

Attend training and meetings as required.

Provide emergency coverage and/or care; report incidents of maltreatment, medical, or behavioral concerns per company policy.

Maintain confidentiality, respect human rights, and practice universal precautions.

Qualifications

High school diploma or equivalent required; Bachelor’s degree preferred.

Two years of experience in human services preferred.

All state‑required training and certification completed within mandated timeframes.

Valid driver’s license in good standing with car registration and vehicle insurance.

Ability to maintain composure and exercise sound judgment and discretion.

Reliable, responsible, and caring nature.

A good listener with empathy while providing guidance.
