CareCollab is a Medicaid care management organization serving high-acuity populations across New York City, Westchester, and Nassau County. We work with individuals who have complex medical, behavioral health, and social needs, including serious mental illness, substance use disorders, chronic conditions, and housing instability.
Through New York’s Health Home program, we provide care coordination services that help members:
access medical and behavioral health care
navigate social services and housing resources
stay engaged in treatment
avoid unnecessary hospitalizations and emergency room visits
Our model combines field-based care management, community partnerships, and a growing technology-enabled infrastructure to deliver consistent, high-quality support to underserved populations. We currently serve over 1,100 members and are building the systems and team required to scale to 3,000–5,000 members over the next two years.
CareCollab is building a scalable enrollment engine, not just a referral network. The Director of Growth & Enrollment will own and drive this effort end-to-end.
You will be responsible for generating 125–200 new member enrollments per month through a combination of:
referral partnerships (hospitals, behavioral health, community orgs)
outbound outreach (call-based model)
field-based enrollment (events, shelters, street outreach)
affiliate pipelines
This role combines strategy, execution, and accountability. Candidates who excel here are comfortable owning outcomes, building processes, and operating in a high-growth environment.
Success requires building and operating a multi-channel enrollment engine across partnerships, outbound outreach, and field-based execution. The Director will be responsible not only for developing relationships, but for driving measurable enrollment outcomes and building the systems required to scale.
This is a high-ownership, fast-paced role where speed, execution, and accountability are critical.
You will own the entire enrollment process including lead generation, outreach execution, conversion to enrollment, pipeline tracking and performance, and alignment with intake, operations and the care management team.
Essential Duties and Responsibilities The Director of Growth & Enrollment will be responsible for building, managing, and continuously improving CareCollab’s end-to-end enrollment engine. This includes full ownership of enrollment growth, with accountability for achieving monthly targets of 125–200 new members and scaling the organization toward 3,000–5,000 members over the next two years.
This role requires developing and executing a multi-channel acquisition strategy that includes referral partnerships, outbound outreach, field-based enrollment, and affiliate pipelines. The Director will be expected to actively build and maintain relationships with hospitals, behavioral health providers, community organizations, and other referral sources, while also ensuring that these relationships translate into consistent and measurable enrollment volume.
In parallel, the Director will design and implement the systems and infrastructure required to support growth at scale. This includes establishing clear processes for referral tracking, pipeline management, conversion monitoring, and outreach performance. Working closely with tools such as Jotform, Microsoft Lists, Power Automate, and VanillaSoft, the Director will ensure that all leads are tracked, no opportunities are lost, and performance is visible in real time.
The role also includes ownership of conversion and funnel optimization. The Director will be responsible for improving referral-to-enrollment conversion rates, reducing time from referral to enrollment, and implementing structured follow-up processes that increase engagement and completion rates.
Team leadership is a critical component of this position. The Director will hire, train, and manage outreach staff, field teams, and other business development personnel, while establishing clear expectations around activity, performance, and outcomes. The Director will directly oversee outreach performance, including call volume, conversion rates, and daily enrollment output across outbound and field teams. This includes building a culture of accountability, where metrics such as calls, enrollments, and conversion rates are consistently monitored and managed.
Finally, the Director will work closely with internal teams, including Start of Care (SOC), Care Management, and Operations, to ensure alignment between growth and execution. This includes maintaining referral quality, ensuring smooth handoffs, and aligning enrollment volume with operational capacity. The Director will also own performance tracking and reporting, using data to drive decisions, identify opportunities, and continuously refine the enrollment strategy.

Consistent enrollment of 125–200 members per month
Fully operational outbound outreach team
Structured and trackable referral pipeline
Improved conversion rates across all channels
At least 1–2 hospital partnerships producing consistent referrals

Who You Are You are a builder and operator, not just a relationship manager. You likely have:
5-10+ years in healthcare growth, BD, or enrollment roles
Proven track record of driving measurable growth and exceeding enrollment or referral targets
Experience building or managing outreach or acquisition teams
Strong understanding of Medicaid, Health Home, or similar care coordination models (strongly preferred)
Demonstrated success building partnerships with healthcare providers and community organizations
You are comfortable being measured against clear performance targets and owning outcomes
Skills
Strong understanding of referral pipelines, outreach strategies, and conversion processes
Ability to operate at both strategic and execution levels
Excellent relationship-building and communication skills
