Shift: 08:00 - 16:30 Days: Monday - Friday, plus one weekend day a month Provide high-quality patient care in a skilled nursing environment. Assist with daily living activities and personal care. Monitor patient conditions and report changes to the healthcare team. Collaborate with interdisciplinary teams to develop and implement patient care plans. Maintain accurate and timely documentation of patient care activities. Background checks and drug screens required.
