GEM Transition Care Center is a 75 bed skilled nursing facility specializing in short-term therapy and dedicated to providing all of our residents with the type of care they deserve.
We offer a variety of therapies and care services to assist patients in regaining their independence, whether they are recovering from a hospital stay or require ongoing medical support. As the Director of Rehab and a valued team member, you will collaborate with therapists, patients, their families and the IDT to guide each patient through a personalized therapy program, helping them reach their recovery goals.

Requirements
CPR Certification (Required)
Licensed Physical Therapy Therapist, Occupational Therapist, or Speech Language Pathologist (Required)
3-4 years of experience in long-term care preferred, along with Rehabilitation Director experience
Good physical and mental health
Ability to perform physical tasks of the position, including lifting unassisted weights of up to 40 pounds
Capable of following oral and written instructions
Possessing training, knowledge, and experience for general supervision of responsibilities
Job Duties
Direct the Rehabilitation Services Staff and manage the Rehab Department
Oversee patient scheduling, ensuring appropriate evaluations and treatments; review medical records and patient charts
Have a foundational understanding of physical medicine, anatomy, and physiology
Provide assessments, treatment, and rehabilitative services to patients recovering from injury or disability; guide residents through their exercises
Adhere to established standards of physical medicine practice
Observe and evaluate treatment effects, recommending changes to physicians as necessary
Conduct appropriate assessments on all patients related to requested therapy and provide reassessments according to policy
Treat patients and their families with respect and dignity
Understand infection prevention and control measures, sterile techniques, and their impact on rehabilitation services
Attend mandatory in-service training at the facility and ensure compliance with facility policies and guidelines among all Rehab staff
Foster positive working relationships within the department and with other departments
Communicate effectively and clearly with physicians, staff, and the administrative team
Handle management duties, including interviewing, hiring, evaluating, terminating, and scheduling departmental staff
Monitor Rehab staff documentation of resident treatments and responses
Assist in developing a teaching plan based on identified learning needs and assess its effectiveness; include families in teaching as appropriate and keep staff and physicians informed of conclusions
Meet current documentation standards and policies
Adhere to the facility’s policies and procedures
