Community Growth Manager – Geriatrics Outpatient Clinic
Location: Field-based / In-Person
Employment Type: Full-time
Compensation: $70,000 - $85,000 + Bonus Potential
Role Overview This role is responsible for supporting the growth of our outpatient geriatrics clinic through community outreach, relationship development, and patient access initiatives.
This is a hands‑on, field‑based role focused on building awareness, trust, and connectivity between the clinic and community partners that serve older adults and their families.

Key Responsibilities Community Outreach & Partnerships Develop and maintain relationships with Assisted Living and Independent Living communities
Develop relationships with primary care and specialty practices
Develop relationships with home health agencies
Develop relationships with community organizations serving older adults
Represent the clinic at community events, educational sessions, and outreach activities
Patient Access & Engagement Educate patients and caregivers on geriatric-focused care
Facilitate warm handoffs to clinic scheduling teams
Support patient access by identifying and reducing logistical barriers
Clinic Growth Support Coordinate with internal teams to align outreach efforts with clinic capacity
Track outreach activity and engagement outcomes
Contribute to initiatives designed to increase clinic awareness and utilization
Reporting & Accountability Maintain accurate outreach activity records
Participate in regular performance reviews and planning meetings
Qualifications 3+ years of experience in healthcare outreach, community relations, admissions, or similar roles
Experience working with older adults, caregivers, or post-acute providers preferred
Strong interpersonal and communication skills
Comfortable working independently in the field
Organized, proactive, and results-oriented
Familiarity with healthcare environments and patient access workflows
