Job Description Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high‑quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion.
We are seeking a relationship‑driven professional to join our team as a Liaison. This role is ideal for someone who thrives on connecting with healthcare partners, educating the community, and helping patients access high‑quality home health services.
This position supports our South Hills and Washington territories and requires regular travel throughout the South Hills region and Washington County.

Responsibilities Build and maintain strong relationships with referral sources, physicians, and community partners.
Serve as a trusted resource on our home health services.
Educate healthcare professionals on patient eligibility and care options.
Identify opportunities to support patient transitions to home‑based care.
Collaborate closely with clinical and operational teams.
Represent Concordia Visiting Nurses with professionalism and integrity.
