Overview: We are seeking a dynamic, community focused individual to lead outreach and engagement efforts across the Hudson Valley region. This is a field-based role that requires a self-starter who can independently plan, execute, and follow up on outreach initiatives. As an organization focused on engagement and outcomes, we leverage nutrition services to re-engage Medicaid members, screen for health related social needs, and connect individuals to critical community resources to improve overall outcomes. In this role, you will serve as a key connector between Medicaid members, healthcare partners, and community based organizations, helping expand access to services and strengthen community partnerships.
Key Responsibilities: Develop and maintain strong relationships with community partners, including FQHCs, clinics, CBOs, providers, and referral sources Drive referral generation and build sustainable partner pipelines through strategic outreach and relationship management Plan, coordinate, and execute community events, workshops, and in-servicing sessions with CBOs, FQHCs, and provider partners Serve as a key point of contact interfacing with clinics, care managers, referral coordinators, and community organizations Own responsibility for generating referrals through community education, outreach, and engagement efforts Educate Medicaid members and community partners on available programs, including eligibility and enrollment processes Represent the organization in the field to increase awareness, engagement, and program participation Collaborate with internal teams to align outreach strategies with organizational and statewide initiatives Identify opportunities to expand partnerships and enhance program reach and effectiveness Track, analyze, and report on outreach activity, referral trends, and program performance metrics
Qualifications: 1–2+ years of experience in community outreach, healthcare navigation, public health, or a related field Preferred experience working with Medicaid populations, FQHCs, CBOs, and referral generation initiatives Strong understanding of community engagement within healthcare or social services environments Excellent communication, relationship building, and organizational skills Ability to work independently in a field based role, with strong planning, execution, and follow up capabilities Proficiency in Microsoft Office or Google Workspace; CRM experience is a plus Bachelor’s degree in public health, social work, or a related field preferred (or equivalent experience) Flexibility to work occasional evenings or weekends for events
