Director of Patient Services
The Director of Patient Services manages all direct care patient services provided by clinical personnel. The role focuses on both home health clinical quality assurance and home health clinical operations projects and reports to the Branch Director.
Salary and Schedule Salary: $103k‑$108k yearly based on experience.
Work Schedule: Full‑time / 40 hours per week.
Position Type: Hybrid.
Branch Location: Liverpool, NY.
Responsibilities
Develop, plan, implement, analyze, and organize clinical operations for a specific location.
Conduct and delegate the assessment and reassessment of patients, including updating care plans and interpreting patient needs, while following company, physician, and health facility procedures.
Manage the assignment of caregivers.
Oversee the delivery of care to all patients served by the location, reviewing case referrals, patient information, and admission criteria, and assigning appropriate clinicians.
Instruct and guide clinicians to promote engaging performance and quality care delivery.
Assist clinicians in establishing therapeutic goals, setting priorities, and developing patient plans of care (POC).
Monitor cases to ensure documentation compliance with regulatory agencies and third‑party payer requirements, including final audits and Medicare compliance.
Coordinate communication among team members, attending physicians, and caregivers to ensure appropriate care and outcome planning.
Work with the Branch Director and Finance Department to establish revenue and budget goals.
Participate in sales and marketing projects.
Oversee all clinical employees assigned to a specific location, evaluate performance, and manage supervisory responsibilities.
Handle employee corrective action and discipline issues fairly, in collaboration with HR and executive leadership.
Participate in interviewing, hiring, and developing direct care clinicians, evaluating performance, coaching staff, and recommending education programs.
Assess staff education needs, conduct reviews of clinical documentation, and deliver regular staff education.
Analyze situations, identify problems, and evaluate alternative courses of action using Performance Improvement principles.
Lead QA/PI activities, work with Utilization Review staff for data tracking, and analyze care outcomes to determine efficiency and efficacy.
Provide direct patient care infrequently and only in emergency situations.
Act as Branch Director in their absence.
Interpret company standards and procedures to ensure compliance with external regulators and internal documentation standards.
Maintain ongoing clinical knowledge through training programs and provide guidance to staff.
Maintain relationships with referral and community sources and participate in professional organizations and care‑related programs.
Required Experience / Skills
Graduate of an accredited School of Nursing.
BSN required (NY state requirement).
Current state license as a Registered Nurse.
Proof of current CPR.
Valid driver’s license, auto insurance, and reliable transportation.
Two years as an RN with at least one year of management experience in a home health environment.
Certified Home Health Agency experience required (NY state requirement) or four years’ CHHA experience and six credit hours toward BSN.
Management and people leadership experience preferred.
OASIS experience required. OASIS certification (HCS‑O, COQS, and/or COS‑C) preferred.
HomeCare Homebase (HCHB) experience preferred.
CMS PDGM billing knowledge and/or experience preferred.
Additional Information
Normal Hours of Operation: M‑F / 8 a – 5 p (ET).
On‑Call Expectation: As needed.
Branch Size: 900 Census (4.5 STAR rating).
Annual Bonus Eligible: Yes, eligible for the annual incentive bonus with payouts both quarterly and annually.
Schedule and Pay Scheduled Weekly Hours: 40.
Pay Range: $77,200 ‑ $106,200 per year. This position is eligible for a bonus incentive plan based on company and/or individual performance.
