Position Title Outreach & Enrollment Representative

Department Outreach

Reports To Operations Manager

Position Overview The Outreach & Enrollment Representative shall approach prospective patients in a linguistically and culturally sensitive manner to achieve FHCCGLA’s new/unduplicated patient goals through engagement, education and promotion of our services.

Duties / Responsibilities

Contact members of the target community (one‑on‑one, door‑to‑door, group sessions, etc.) and offer all FHCCGLA services, including clinic services (primary care, behavioral health, dental, optometry “coming soon”, family planning, immunizations, OB/GYN, etc.), breast health care, reproductive health care, and application assistance for My Health LA, Covered California, Medi‑Cal, and Medicare.

Meet set goals for appointments scheduled for new (unduplicated) patients and refer patients to clinic sites that are open to new patients.

Outreach various target populations such as teens, family PACT‑eligible clients of contraceptive age (women 12‑55, men up to age 60), pediatric patients, prenatal patients, and others.

Create and distribute flyers and other materials to the target population and throughout the community.

Participate in scheduled activities including community events (health fairs, toy give‑aways, etc.), presentations, table/booth set‑up (at grocery stores, immigration offices, car washes, etc.), and immunization activities (such as flu clinics).

Contact patients via phone calls, postcards, or letters for Medi‑Cal re‑determination, Initial Health Assessments (IHAs), Clinical Care Measures, managed care patients returning for a clinic visit, and patient retention efforts (surveys).

Assist with the distribution and collection of the Patient Satisfaction Survey (annually) and Community Needs Assessment (every 5 years).

Track and follow‑up on all outreach referrals and contacts made by the outreach team, including dropping off and picking up referral pads, calling prospective patients to remind them of their appointment, and re‑scheduling no‑shows as necessary.

Document all follow‑up contact efforts.

Assist in securing relationships with various community‑based organizations and identify, initiate, and implement measures to deliver high‑quality care and improve services.

Respond efficiently and timely to all patient and provider staff needs and inquiries and ensure excellent customer service.

Work with the operations manager to manage patient scheduling and flow to address bottlenecks and scheduling issues.

Handle patient grievances according to FHCCGLA’s policies and procedures.

Assist in developing, updating, and reviewing FHCCGLA policies and procedures (P&P’s) as needed.

Ensure HIPAA compliance by maintaining strict confidentiality of all patient data and the EHR/PMS system according to regulations.

Attend mandatory quarterly staff meetings/training, corporate risk management meetings, clinical & operations meetings, meetings with executive leadership, and other pertinent meetings as scheduled.

Remain informed of current legal and regulatory changes related to scope of practice, specific programs/payors, insurances accepted, services offered, policies, and all other relevant updates.

Perform other duties as assigned.

Minimum Qualifications

High school diploma or GED equivalent.

Exceptional organizational and analytical skills.

Computer literacy (Word, Excel, PowerPoint). Knowledge of NextGen (EHR & PMS) and Microsoft Office preferred.

Experience working in a FQHC highly preferred.

Excellent written and verbal communication skills.

Knowledge of administrative principles and procedures.

Ability to speak publicly and present.

Bilingual English/Spanish with familiarity with Hispanic culture; Spanish speaking preferred.

Access to automobile with a valid California driver’s license and state‑mandated automobile insurance.

Work schedule may include evenings, overtime, and weekends as needed.

Ability to prioritize workload, work under pressure, and meet tight deadlines.

Motivated and committed to providing high‑quality healthcare for indigent and underserved communities.

Willingness to adapt to changes related to the agency’s growth and expansion.

Ability to function independently with little supervision.

Presentations

Prepare and create educational PowerPoint presentations and health education materials for the target community.

Use visual aids and other tools to inform the public effectively.

Retain sign‑in sheets and evaluation forms as necessary.

Schedule and conduct presentations at community organizations, school settings, work sites, and social service agencies on topics such as clinic services, payors/programs available, Family PACT, VFC immunizations, STI education, and other health‑related topics.

Other Duties

Assist in the preparation of reports related to outreach and grant activities.

Attend continuing education conferences, workshops, and in‑services as scheduled or required to maintain up‑to‑date knowledge.

Maintain a calendar and record outreach activities, contacts, presentations, and education provided to ensure goal attainment.

Maintain orderly files, reports, activity logs, and documents for immediate retrieval.

Submit orders to purchase incentives, visual aids, or supplies as budget allows through agency funds or grants.

Work well in a professional, team‑oriented environment with strong interpersonal skills.

Deliver excellent customer service and communication.

Willingness to travel and possess the knowledge of My Health LA, PPOs, HMOs, Medicare, Family PACT, CPSP, Medi‑Cal, Fee‑for‑Service, CHDP, Managed Care Plans, and Every Woman Counts.

Provide translation services as necessary.
