Overview
The Substance Abuse Treatment Program [SATP] at Montefiore has been providing methadone treatment for opioid dependence for over 35 years. SATP Units I and III offer cutting‑edge individual and group counseling to admitted patients. The Outreach Specialist will assist individuals enrolled in treatment at SATP in engaging in their substance abuse treatment. The associate will aid patients in the program by obtaining abstinence and complying with scheduled appointments and will help patients engage and comply with their onsite and offsite primary care and psychiatric treatment. The Outreach Specialist will aid in initial patient engagement, outreach efforts and accompanying patients to appointments as needed.
Responsibilities
Work closely with patients enrolled in SATP and NDRC to aid in engagement in treatment. This may include meeting with patients within the program, calling patients by phone, meeting with patients in the community, and accompanying patients to appointments. Outreach patients who have stopped attending treatment at their last known residence.
Actively listen to patients enrolled at SATP and NDRC and encourage them to communicate any concerns to appropriate staff on their treatment team. Major issues will be escalated to supervisors.
Document all patient contacts in the electronic medical record, including phone calls, onsite visits and offsite interactions.
Meet with patients in emergency department to assist in engagement and referral to substance abuse treatment at SATP and NDRC.
Assist individuals enrolled in treatment with referrals to needed services and connection to resources in the community.
Describe the nature and purpose of services offered at SATP and NDRC to patients interested in or enrolled in the program.
Motivate patients enrolled at SATP and NDRC to actively participate in their health and recovery.
Collaborate with a multidisciplinary team in order to build and maintain positive working relationships with staff at SATP and NDRC.
Requirements
High School Diploma/GED required.
Certified Recovery Peer Advocate (CRPA) or CRPA‑eligible following completion of internship hours required.
Familiarity with Microsoft Word and Excel.
Excellent oral, written and interpersonal communication skills.
Ability to provide outreach to patients enrolled in program, find creative solutions to challenges, build motivation in patients using empirically supported strategies including motivational interviewing.
Ability to demonstrate ethical behavior and uphold standards, to provide emotional support and instrumental assistance to patients enrolled in treatment, and to assist patients in defining and working toward their recovery goals.
Utilize knowledge of adults across the lifespan to help develop patient‑specific goals and provide patient‑centered services.
The schedule is Mondays, Wednesdays, Fridays 7:00 am – 3:30pm @ 3550 Jerome Avenue and Tuesdays and Thursdays 8:30am – 5:00pm @ 2058 Jerome Avenue.
This is a 100% grant funded position.
