Schedule & Location This full‑time position will be based at our housing site in Collinsville, IL. The successful candidate will work Monday through Friday from 3:00pm to 11:30pm.
Pay The anticipated starting pay for new hires for this position is between $19.00 and $20.00 an hour.
Responsibilities
Provide services where they are most effective—meeting clients in their homes or other community settings. You will offer initial triage and support during crisis situations, ensuring a rapid and grounded response.
Utilize the IM-CANS assessment tool to co-create consumer‑driven, personalized care plans. You’ll ensure all identified services meet program requirements while serving as a roadmap for the client's specific recovery goals.
Conduct individual, family, and group therapy sessions using Evidence‑Based Practices (EBPs). Your focus will be on education and skill‑building to enhance client autonomy and daily living capabilities.
Act as a dedicated advocate by providing case management, medication training, and consistent monitoring. You will help clients navigate the complexities of the healthcare system and understand their treatment regimens.
Manage the essential “behind‑the‑scenes” continuity of care, including the pre‑authorization and re‑authorization of services to ensure treatment remains uninterrupted.
Promotion of the Recovery Model Champion a trauma‑informed approach to care that prioritizes:

Qualifications
Candidates for this position must possess a high school diploma or equivalent and have five years of supervised mental health experience.
Alternatively, a bachelor’s degree in counseling, social work, education, psychology, or a related human services field is required.
Individuals with a bachelor’s degree in any other field must have at least 2 years of supervised clinical experience in a mental health setting.
Demonstrated skills in human services and in services to adults.
Demonstrated ability to work within organizational structure; to accept supervision; and to work constructively with consumers, treatment resources, and the community.
Basic computer skills including MS Word and an electronic medical record.
Valid driver’s license, private auto insurance, and be insurable.
