Description

Purpose:

To ensure staff assist the individuals AIM serves to achieve their personal goals while promoting a sense of self confidence and well-being. To support and supervise Direct Support Professionals providing Residential Community Habilitation. To provide training, evaluations, and Residential Community Habilitation program implementation within the service setting. To ensure documentation is completed and service provisions have been met. To provide role modeling, observations and leadership to staff proving Residential Community Habilitation Services.

Essential Functions

Manage the day-to-day operations of community habilitation services for people in certified residential settings.

Author Residential Community Habilitation plans based on individual wants, needs, and goals. Remain responsive to the individual’s unique and changing needs.

To be able to teach and model Direct Support Professionals how to work on a specific goal outlined in the Residential Community Habilitation plans and offer support as needed. The QIDP will teach methods, responsibility for program implementation, and the type of data and frequency of collection.

Assess, evaluate, and author safeguards in Residential Community Habilitation plans. Ensure continuity of safeguards across all service environments.

To ensure that adequate environmental supports and assistive devices are present to promote independence.

Oversee, and ensure the implementation of person centered planning, community mapping, and CQL Personal Outcome Measures in daily services.

Ensure all services are properly documented and billed in the appropriate time frame.

Train staff in service-specific duties and needs, and review all plans for each person served.

Implement, and ensure staff's implementation and documentation of each person's plan, including participation in circle of support meetings as requested/required by the person.

To work in a collaborative manner with the people served to assure effective and efficient staffing in conjunction with the needs of the program(s) and budgetary constraints. Ensure that DSPs are cross-trained for programs within the assigned group.

Perform medical/health related functions adhering to AMAP Policy.

Operate agency vehicles in compliance with agency policies/procedures, and traffic laws, assuring documenting vehicle maintenance/condition. To provide for/assure the appropriate use/care of individual/agency equipment.

Assure people’s personal needs are met, tailored to individual preferences.

Assign and maintain staff schedules.

Keep schedule flexible to ensure contact and observation of all staff while on duty, ensuring consistency in supports.

Other duties

Committee membership as assigned.

Any and all other duties requested by supervisor.

On-Call duty as assigned.

Responsible To The Residential Director or designee

Qualifications

Bachelor’s degree preferred; 3 years experience in ID/DD or related field

1 year managerial experience preferred

Knowledge of applicable Regulations

Willingness to maintain a flexible 40 hour work week

CPR/First Aid, AMAP, and SCIP Certification (Training Provided)

Valid NYS Driver's License acceptable to Agency insurance standards

Ability to work independently, strong organizational and decision-making skills

Proficiency in Microsoft Word and Excel

Demonstrated ability to communicate effectively in oral and written form; excellent verbal, written and presentation skills

Well developed team skills, integrity, and ability to handle interpersonal relationships and sensitive issues

Physical/Cognitive Requirements

Sitting and/or computer use for extended periods

Bending, reaching, pushing, and pulling; ability to safely lift a minimum of 50 lbs

Ability to facilitate desired outcomes of the individuals served while adhering to agency standards and policies and state regulations

Maintain training and certification in all required areas

Ability to remain calm in stressful situations

Ability to maintain confidentiality

Salary $22.25-$28.25
