Director of Community Relations (Emerging Leader Opportunity)

What You’ll Own
This role sits at the intersection of community, operations, and growth.

Responsibilities

Build and deepen relationships with referral partners (home health, hospice, physicians, senior living, etc.)

Represent our agency in the community with credibility and consistency

Create a steady pipeline of new client referrals

Identify opportunities to expand partnerships—not just maintain them

Collaborate closely with our internal team to ensure a strong handoff from referral → client experience

Track what’s working, what’s not, and adjust quickly

You are responsible for results, not just activity.

Who This Is For

A Care Manager or Care Coordinator who understands families and wants to grow into a more strategic role

A Social Worker (MSW/LCSW) who enjoys relationship‑building and wants to expand beyond direct service

Compensation

Competitive base salary $85,000‑$105,000

Performance‑based commission tied to growth
