We are seeking an experienced Registered Nurse (RN) to join our team as a Hybrid UAS Nurse serving Albany and Schenectady Counties, NY. This role offers a consistent weekday schedule and a hybrid work model that combines in-home member assessments with remote documentation.

Job Function:
- Day Shift.
- 9am - 5pm.
- Perform in-home UAS-NY assessments throughout Albany and Schenectady Counties
- Conduct comprehensive nursing assessments including medical, functional, and psychosocial evaluations
- Complete UAS documentation accurately and within required timeframes
- Utilize electronic documentation systems efficiently
- Communicate findings with care managers, providers, and interdisciplinary teams
- Maintain compliance with NYS Department of Health regulations and organizational policies
- Independently manage daily scheduling, travel, and territory coverage
- Active New York State Registered Nurse (RN) license
- Prior UAS-NY assessment experience required
- 2+ Years of Home Care experience
- Strong clinical assessment and documentation skills
- Comfortable working independently in home and community settings
- Reliable transportation and willingness to travel throughout Albany and Schenectady Counties
- Proficient with electronic health record systems
- Organized, professional, and dependable
