Purpose
To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.
Qualifications
Must successfully complete the required pre-employment Background and Drug Screenings
Must possess a valid Driver’s License which meets company standards for approved driver status
Must possess advanced computer skills
Must possess a High School Diploma or equivalent, Bachelor’s Degree preferred
Experience working in the field of intellectual/developmental disabilities preferred
Minimum one year of experience in writing program plans, case notes or other relevant documentation required
Must be a mandated reporter and able to report any instances of abuse, neglect, or exploitation that they are involved in or witness.
Essential Functions
Responsible for the development, implementation and coordination of individual’s program plans
Develop, implement and monitor individualized training programs to meet identified priority training needs
Ensures all plans are person‑centered and reflect the individual’s hopes and dreams through active participation wherever possible
Ensure all documentation is accurate and complete according to regulatory requirements
Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed
Perform other duties as assigned
All candidates for employment must submit to a pre‑employment drug screening after receiving a conditional offer of employment. A positive confirmed screening, or refusal to submit to such screening, will result in a withdrawal of the conditional job offer.
