Your responsibilities include:/ppRelationship Building

Outreach/pulliMeet regularly with physicians, hospitals, discharge planners, social workers, senior ministries, and other healthcare professionals/liliPromote awareness of our services and Medicare home health guidelines/liliConduct community education and public speaking engagements/li/ulpProgram Development

Quality Support/pulliResearch, design, and implement new programs to enhance service quality/liliAddress concerns from clients, families, and healthcare partners/liliMonitor quality assurance issues and communicate updates/li/ulpPlanning
