Position Overview We are seeking a motivated and relationship‑driven

Home Health Marketer / Community Outreach Liaison

to grow and maintain referral relationships across the NOVA region. This role focuses on increasing patient referrals, building partnerships with healthcare providers, and representing ECVHH in the community. You will be the face of the agency to physician offices, hospitals, clinics, case managers, and senior‑care organizations.

Key Responsibilities Referral Development

Identify, build, and maintain strong relationships with physicians, clinics, hospitals, assisted living facilities, rehab centers, and case managers.

Conduct regular in‑person visits, calls, and follow‑ups to generate referrals.

Present ECVHH services clearly and professionally to staff and providers.

Track referral sources and ensure consistent engagement.

Community Outreach

Represent ECVHH at community events, health fairs, senior‑care programs, and networking events.

Organize and participate in outreach activities to increase brand awareness.

Deliver marketing materials and maintain an updated supply (flyers, brochures, business cards, etc.).

Agency Support

Collaborate with the office team to ensure prompt follow‑up on new referrals.

Communicate patient needs, provider feedback, and market trends to management.

Maintain accurate logs of visits, activities, and referral pipeline.

Compliance

Understand CMS, DMAS, and payer guidelines related to home health referrals.

Ensure all marketing activities comply with healthcare regulations (no inducements, no gifts, etc.).

Follow agency policies and HIPAA requirements.

Qualifications

Previous experience in home health, hospice, DME, senior care, or healthcare marketing strongly preferred.

Strong communication, relationship‑building, and interpersonal skills.

Ability to learn clinical services and explain them in simple terms.

Reliable transportation for daily field travel.

Highly organized, self‑motivated, and results‑driven.

Bilingual a plus (but not required).

Work Schedule

Flexible schedule; majority of work performed in the field.

Weekly or bi‑weekly check‑ins with ECVHH leadership.
