Overview
This position is a per diem rotating position for LEADER Men's Residential Program. LEADER is a program for Law Enforcement, Active Duty Military and Emergency Responders. Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice.
Essential Functions
Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
Provides and supervises community activities according to needs.
May include driving & transporting patients to and from activities in hospital vehicles.
Remains awake and alert at all times while on the job.
Assists patients in maintaining and keeping various appointments.
Clear, concise, and timely documentation and shift notes.
Establishes relationships with patients that are helpful and maintains professional boundaries.
Participates in patient rounds, status checks, or teams per program guidelines.
Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
Works effectively to manage the therapeutic milieu.
Provides patients with direction and supervision as needed.
Follows established hospital & program policies, procedures, and protocols.
Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
Encourages patients in their activities of daily living.
Assesses sleep patterns and assists patients to develop more appropriate sleep cycles, notifying clinical team as necessary.
Completes mandatory annual in-service requirements, including but not limited to: CPR and de‑escalation.
Attends and participates in rounds and case conferences when appropriate.
Physical ability for participation in patient activities.
Other duties as assigned.
Education High School Diploma or Equivalent required and Bachelor's Degree Related Field of Study preferred.
Can this role accept experience in lieu of a degree? No.
Licenses and Credentials Medication Administration Program Certified [MAP – Massachusetts] – Massachusetts Department of Public Health required.
Experience One‑year patient care or related experience preferred, preferably in a psychiatric setting 1–2 years preferred.
Knowledge, Skills and Abilities
Driver's license (MA, CT, NH, NY, RI, VT) is required with the ability to satisfactorily meet and maintain hospital driving standards.
Time management.
Excellent verbal and written communication skills.
Ability to work as part of a team.
Maintains a commitment to professional growth and competency.
All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.
Familiarity with Epic preferred.
