Job Summary

Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in a therapeutic milieu within the scope of defined practice. Does this position require Patient Care? Yes.

Essential Functions

Provides direct patient care in accordance with current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator and Charge Nurse.

Provides and supervises community activities according to needs.

May include driving & transporting patients to and from activities in hospital vehicles.

Remains awake and alert at all times while on the job.

Assists patients in maintaining and keeping various appointments.

Clear, concise, and timely documentation and shift notes.

Establishes relationships with patients that are helpful and maintains professional boundaries.

Participates in patient rounds, status checks, or teams per program guidelines.

Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.

Works effectively to manage the therapeutic milieu.

Provides patients with direction and supervision as needed.

Follows established hospital & program policies, procedures, and protocols.

Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.

Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.

Encourages patients in their activities of daily living.

Assesses sleep patterns and assists patients to develop more appropriate sleep cycles, notifying clinical team as necessary.

Completes mandatory annual in-service requirements, including but not limited to CPR and de‑escalation.

Attends and participates in rounds and case conferences when appropriate.

Physical ability for participation in patient activities.

Other duties as assigned.

Qualifications Education High School Diploma or Equivalent required; Bachelor's Degree Related Field of Study preferred. No experience in lieu of a degree.

Licenses and Credentials Medication Administration Program Certified (MAP – Massachusetts) required.

Experience One‑year patient care or related experience preferred, preferably in a psychiatric setting; 1‑2 years preferred.

Basic computer skills for electronic or online systems (PRISM) and communication, clinical and administrative purposes.

Familiarity with Epic preferred.
