Manager I Care Management - Cherokee/Clay/Graham CFSP Elevance Health is seeking a Manager I Care Management to operationalize a statewide Medicaid Plan focused on supporting Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system. This field‑based role is crucial for ensuring seamless, integrated, and coordinated health care across all counties of Cherokee, Clay, and Graham.

Location:

Cherokee, Clay, or Graham County – must reside in these counties and cover all three locations. Field position with virtual work when not in the field.

Hours:

General business hours, Monday through Friday.

Travel:

Travel within assigned areas is required.

Responsibilities:

Manage resource utilization to ensure appropriate delivery of care to members.

Review all Care Plans and ISPs for quality control and provide guidance on addressing members’ complex health and social needs.

Ensure care managers provide Trauma‑Informed Care and recognize the impact of ACEs on the CFSP population.

Coordinate service delivery, conducting member assessments of physical and psychological factors.

Participate in cross‑functional workgroups created to maintain and develop the program.

Evaluate current processes of Special Program support functions and recommend changes for increased efficiencies and improved outcomes.

Develop and conduct training programs for staff involved in the program.

Extract and manipulate analytical data to present findings to relevant markets and stakeholders.

Hire, train, coach, counsel, and evaluate performance of direct reports.

Required Qualifications:

Active and current license as an LCSW, LCMHC, LMFT, or RN issued by the state of North Carolina.

MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.

Three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.

Preferred Qualifications:

Knowledge of resources, supports, services and opportunities required for safe community living for populations receiving in‑reach and transition services, including LTSS, Behavioral Health, therapeutic, and physical health services.

Experience working with Children, Youth, and Families served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs.

At least 2 years of management/supervisor experience with direct reports.

Service delivery coordination, discharge planning or behavioral health experience in a managed care setting.

Additional Information:

North Carolina residency is required.

LCSW‑A, LCMHC‑A or other associate level licenses are not accepted.
