Join Trinity Health in the role of RN Case Manager at our Eddy SeniorCare location in Latham. This part‑time position focuses on community‑based home visits and coordination of care for PACE participants to ensure optimal health outcomes and quality of life.
Position Summary We are seeking a compassionate and experienced RN Case Manager to join our interdisciplinary team. This role focuses on providing community‑based home visits and coordinating care for PACE participants to ensure optimal health outcomes and quality of life.
Responsibilities
Conduct in‑home assessments and develop individualized care plans.
Collaborate with physicians, social workers, and other team members to manage participant needs.
Provide education and support to participants and caregivers.
Monitor clinical status and adjust care plans as needed.
Document all care activities in compliance with regulatory and organizational standards.
Qualifications
Current NYS RN license in good standing.
Minimum 1 year of nursing experience (homecare or community health preferred).
Strong communication and organizational skills.
Ability to work independently and travel within the service area.
Work Schedule
Weekday hours, 8 am to 4:30 pm.
24 hours per week (3 shifts).
Pay Range $36.50 – $49.95 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
