NYS Home Health Aide Certificate Proof of eligibility to work in the U.S. Valid driver's license (preferred) Recent physical exam (within 1 year) PPD or QuantiFERON (within 1 year) or chest X-ray (within 5 years) MMR immunizations within the last 10 years
